STRIVE Transition Program

The STRIVE Transition Program, located at Prairie Ridge High School, is a comprehensive post-secondary transition program designed to support young adults ages 18–22 in developing the skills needed for independent adult living. The program provides meaningful, developmentally appropriate learning experiences within a structured, supportive environment while promoting independence, confidence, and real-world skill application.

Students participate in a combination of classroom-based instruction and community-based experiences focused on employment, recreation, daily living, and social engagement. Through authentic, hands-on opportunities, students build practical skills they can use in their homes, workplaces, and communities. Experiences take place both within the transition program setting and throughout the community, allowing students to practice skills in real-world environments with increasing independence and adult expectations.

The STRIVE program focuses on the development of vocational and career skills, independent living, community integration, safety and wellness, self-advocacy, and functional academics. Individual goals are guided by each student’s transition plan and are supported through collaboration between staff, families, and community partners.

Students receiving special education services may be eligible to continue receiving services through District 155 until the day before their 22nd birthday. Transition planning begins at age 14½ and focuses on postsecondary outcomes related to education and training, employment, and independent living. Throughout high school, students participate in transition assessments, career exploration, and skill development activities to help guide future planning. If a student demonstrates significant needs in transition skill areas, the IEP team may consider participation in the STRIVE program or additional transition supports, typically beginning no later than the first semester of the student’s junior year.

The transition planning and program decision-making process is collaborative and includes the student, parents/guardians, teachers, case manager, related service providers, school counselors, and, when appropriate, adult service agencies.
